Details:Adding custom fields to MIDAS with specific names and setting these fields to appear on Step 4 of the Public Booking Request process may prevent requestors from submitting their requests
Steps to Reproduce:
  1. Create a custom booking field named "email" and set it to appear in Web Requests. Then try to make a booking request
Workaround:Remove, Rename, or set not to show on Web Requests any custom booking field named "email" or "org"
Resolution:Fixed for v4.09

Details:Attempting to approve booking requests form the Pending Booking Requests screen simply reloads the screen, and doesn't approve the request
Workaround:Instead of clicking the "Approve" icon, click the "Modify Request" icon, and approve the request that way.
Resolution:Fixed for v4.06 if downloaded/updated after 4th May 2014
